Garda Vetting Disclosure

Every Healthcare applicant must complete a Garda Vetting Application form. A candidates' application cannot be processed until this form is fully completed. Please take note of the following before completing the application form:

1. Please use BLOCK CAPITALS.
2. Ensure your name is spelt correctly.
3. Include every single address you have lived at since you were born.
4. Fill out as far as and including the declaration of application (YOU) but not beyond that.
5. Please return the application form to: Servisource Recruitment, Quayside Business Park, Mill Street, Dundalk, Co. Louth, Ireland with a postal order to the value of €11.00
6. Please note incomplete forms will be returned and you will be asked to resubmit with another €11.00
7. Garda Vetting Disclosures can take up to 6 weeks to be processed.
8. Garda Vetting Disclosures are confidential and are under the Data Protection Unit and therefore will not be available for inspection at any time to any person's other than authorised by the Garda Central Vetting Unit.
